What should I bring to my first appointment?

Please bring your completed forms ( or arrive early to fill them out), a valid photo ID, your insurance card, a list of your chronic medical conditions and/ or important previous medical records, and all your current medications.

Do I need a referral to see a provider at Zunimed?

Not usually. Most patients can book directly with us.

Do you offer same – day appointments?

We do our best to accommodate urgent needs, please call our office to check availability.

How can I request a prescription refill?

Call our office during business hours or request through pharmacy. Please allow 24 – 48 hours for processing.

About Late arrivals

At Zunimed, we respect our patients’ time and strive to stay on schedule so that everyone receives the attention and care they deserve.

Please plan to arrive at least 10-15 minutes before scheduled appointment time, especially if you need to complete forms.

If you arrive more than 5 minutes late, we may have to reschedule your appointment to avoid impacting other patients.

About Cancellations & No – Shows

Is you need to cancel or reschedule your appointment, please notify us at least 24 hours in advance.

This allows us to offer your time slot to another patient who needs care.

Repeated late cancellations or missed appointments (no-shows) may results in a fee and/or require a deposit to book future appointments.

We understand that unsuspected situations happen, and we’ll do our best to accommodate you. Please call our office as soon as possible if you are running late or need to change your appointment.
